As part of our ongoing compliance, Fresh Hope Care are required to record information about all visitors and NSW Health have recommended electronic collection of names and contact numbers should tracing be required. By Friday 20 November, all Fresh Hope Care facilities will require visitors to scan the QR Code before entering the facility.

To make this work, we ask each visitor to download the Service NSW app. This is the preferred and most secure QR code system that we can implement. If you do not already have the app, it is available for iOS or Android mobile phones via the App Store or Play Store. Using the app’s COVID Safe Check-in tool, there will be a QR code for scanning at the entrance and then it’s as simple as following the prompts. Once you have downloaded the app onto your phone and scanned in once, it will remember you and will automatically come up with your details next time you scan to enter the facility.

The information provided will not be stored by Fresh Hope Care. It will be stored by NSW Health for 28 days and will only be used for contract tracing if required.
